There is nothing wrong, it is normal function of smartword4TFS, please review the below image.
Once you are connected to the team project smart panel appears on the right hand side. If you close it from the "x" button, you can always bring it back by clicking on the "Show Process Template" button.

Yes this issue is fixed in 18.104.22.168 and this version also supports TFS2013 Update 2.
We have recommended to use TFS2013 Update 3 as there are some fixings done by microsoft for the image rendering in the rich text field.
Let me know your feedback once you deploy this latest version.

To perform all function user must have contributor’s right on TFS. In following screenshot SmartWord is connected TFS with TFSR (user with reader’s rights) It got able to fetch a work-item on document using “Get Work-item” function, it’s also able to execute queries as well. When you modify the work-item using “Edit work-Item” it didn't save changes also at the time of publishing modified work-item from document it prompted the error message as shown in 2 nd image.
So user must be Contributor to perform all operations in SmartWord including publish and SmartWord also allows Readers to get work-item and execute queries.
Documents Authoring and publishing requires Contributer rights.
