Hi,
Yes, you can embed a Table in the Description Field and then make it as a SmartPart, so whenever you want to add a PBI just call the that SmartPart and you will get the Table in the Description Field by Default.
Yes, you can also define the Smart Part in such a way that some of the attributes will be in the Table Layout while some in the Form Layout.

Yes this can be done. Just add the table to the Description field and create a Smart Part. Click "Yes" when asked if you need to keep the contents of the Smart Parts.
Also be careful about what you include in the Description field because if the Work Items are accessed via Visual Studio, there is a chance that formatting might be affected. I included a table in my first template and it didn't really come out like I expected it to.
My advice to you is to keep it simple while utilising the powerful features of SmartWord4TFS.
