Hi,
In the DOTX or Template Mode you didn't get all the options of SmartWord. But still the Smart Panel should appear and also other options like Create Smart Part, Insert Smart Part, Wizard, Queries and Control Properties are enabled.
Yes but you can't Publish or Fetch the Work Items unless you link\connect with the Team Project.
And if you open a new document based on the Word Template then the Create Smart Part and Wizard options also get disabled.
See the following image for clarification:

My question exactly, the Smart Panel doesn't appear. So I can't create any Smart Parts to be used as a template in future. Can you please give me step by step instructions on this using Word 2013? I'm worried that I'm not doing something correctly. Maybe it has nothing do to with SmartWord4TFS, but something I'm doing correctly in Word. Can you please give me the steps?

Hi Ramila, I created a .DOTX file and noticed that after connecting the newly created document to TFS, clicking on Wizard and applying the desired template will load it back into the Insert Smart Part button. Hope this helps.
