Yes, you can hide\disappear the Empty Fields that show up in the original grey text as "Feature Description" Or "Requirement Description".
I have created a small video in this regard which is present in the attachment. Kindly extract and watch this video.

Secondly, If you do the "get workitem" with the SELECT RESULT TYPE = FLAT LIST then it will show the work items in the Flat List, so for the Hierarchy of Work Items first create the SmartPart in the Hierarchical Structure and then while doing the "get workitem" SELECT the RESULT TYPE = TREE OF WORKITEMS.
Kindly perform as i suggested above and do let us know that is it get better or not?

In the future, would be helpful to say "Ribbon > Document Authoring > Control Properties > Delete PlaceHolderText" for those unable to open the attachment.

The issue is that I am using the wizard and a query to generate an entire requirement document with hundreds of work items.
I cleared the Placeholder field in the wizard, but this is not being applied when the document is generated. I tried modifying the fields to remove the placeholder, auto create smart parts, then refresh, and placeholders still return

Manually changing every description to clear the placeholder is not an option, and will get reset every time I refresh the document.

The placeholder text is returning\showing in the document if we delete it from the Wizard. But if you manually remove it from the Control Properties and then do the auto create smart part then on performing the GetWorkItem or running of the Queries, then for the empty description fields the placeholder text doesn't appear and it is working fine at our end too.
But as you mentioned that it is not working at your end its bit strange, So for that can we please do a brief GoToMeeting session tomorrow where I will investigate the issue and try to resolve it too.

Unfortunately due to my work environment, I cannot allow a remote session.
I will make a ton of screenshots documenting my work flow, to help you replicate the issue.
I tried auto create smart parts, and using the "get workitem" smart panel, and behaviors were not what I expected (inserting fields that I don't want such as iteration/area, Making it in a flat list instead of hierarchy, etc)
I have been most successful by defining the hierarchy and fields in the wizard (as shown in the above image), then using a TREE query that I have defined.
Will step by step screenshots be sufficient?
