No Audit/ traceability on requirements within the team
Anybody running into Audit issues where other users on your team issue requirements for approval and there is no oversight on what has been approved or comments made on them?
There is no transparency, no visibility across the team.
When a team member moves on there is no way to see any of this and in a regulated environment this is a disaster - there's no traceability on comments, why changes were made and worst of all, for historical items there is absolutely no audit trail on the actual approvals (i.e. requirements approved prior to the very recent change to update approvals into the work item discussion thread)
